<p>The victim hit her head at the bottom of the slide because it had partially deflated. fractured her neck going down the water slide which had been placed over the concrete edge of a pool.</p>
<p>Two other serious injuries have occurred in a similar manner: a 24-year-old man from Springfield, Mo. became a quadriplegic and a woman from Allentown, Pa. fractured her neck.<span id="more-35259"></span></p>
<p>Not only can the slide can deflate, putting the user at risk of hitting the ground underneath the slide, it is also unstable and can topple over in both still and windy conditions and carries inadequate warnings and instructions.</p>

<p>The recall product was made in China by Manley Toys Ltd and is decribed as: &#8220;&#8230;[having] a blue base, yellow sliding mat and an arch going over the top of the slide. By connecting a hose to the top of the slide, water can be sprayed on its downward slope. The words &#8216;Banzai Splash&#8217; are printed in a circular blue, orange and white logo, shaped like a wave on either side of the slide.</p>
<p>The recalled slides were sold at Walmart and Toys R Us in the USA from January 2005 until June 2009 for about $250. The recalled slides have the barcode number 2675315734 and model number 15734. Both the barcode and model number appear on the original packaging but are not on the actual slide.</p>
<p>Owners of this product are urged to stop its use immediately and return it to the nearest Walmart or Toys R Us for a full refund. Consumers can also cut the two safety warning notices out of the slide and just return that portion.</p>

<p><strong><em>UPDATE 4th May 2012 :</em></strong> <em>a recall on this product has also been published in Great Britain. Stop the use of this toy immediately; return it to the original place of purchase for a refund. <p>Around 3,000 units of this product in the United States and 150 in Canada are affected by the recall. The rattle toys were made in China.</p>
<p>Whoozit Starry Time Rattles were sold in a variety of specialty toy and baby stores in the USA and Canada. It was also sold via Manhattan&#8217;s own website: www.manhattantoy.com. Sold for about $15, the toy was available from September 2011 until March 2012.</p>
<p>The recalled toy is described by the US Consumer Product Safety Commission (CPSC) as: &#8220;plastic rattles feature three colored [sic] (orange, aqua blue and purple) stars stacked front to back between two clear spheres on a flexible stem. The rattle measures about 5-1/2-inches long. Each clear ball, located at the end of the rattle, measures 1-3/4 inches in diameter. The clear rattles contain small multi-colored [sic] beads and a white plastic disc featuring a blue smiling character face inside. The name of the product is printed on the hang tag.&#8221;</p>
<p>The affected rattles should be taken away from young children immediately and returned to the store where it was purchased for a full refund.</p>

<p>Dorel, the products importers, has received 200 reports of locks that did not adequately secure the cabinet, including reports of damaged locks. From the reported incidents there were 140 children between the ages of 9 months and 5 years who were able to disengage the locks and gain access to the cabinet’s contents.</p>
<p>In three of the reported incidents, the children who gained access swallowed or handled dishwashing detergent, window cleaner or oven cleaner, and were treated, observed and released from emergency treatment centres.<span id="more-31279"></span></p>
<p>The recall involves Safety 1st Push ‘N Snap cabinet locks with model numbers 48391 and 48442. The model numbers are printed on the back of the product and on packaging. The locks are used to secure cabinets with two straps that wrap around the knobs or handles on a cabinet door. When the product is in the “lock” position, a green triangle is shown through a window on the device. The Safety 1st logo is embossed on the front of the lock. Locks that were manufactured between January 2004 and November 2010 are included in the recall. The date of manufacture is embossed on the back. The arrow on the date wheel points to the month and the numbers of either side of arrow represent the year of manufacture.</p>

<p>The locks, that were made in China, were sold at Bed Bath &amp; Beyond, and other retail stores nationwide and online at Amazon.com from January 2004 through February 2012 for between $2 and $4.</p>
<p>Use of these locks should stop immediately, Dorel Juvenile Group can be contacted for a for a free replacement Push ‘N Snap lock (with model numbers HS158 or HS159). When removing the recalled locks, consumers are urged to immediately store dangerous items out of reach of children.</p>

<p>If the security screws are not re-fitted, then it is possible that a gap may form between the lower and upper cot bed end sections if the cot is moved or lifted for any reason. This could cause the possibility of injury to parent or child. It is also possible that, in some cases, partial disassembly of the drop side mechanism could occur, rendering the cot unsafe.</p>
<p>If the cot bed is converted to a sofa, as stated in the instructions, it is also important that each of the security screws is re-fitted.</p>

<p>The product was made in China. Around 1,800 units in the United States and 350 in Canada are affected.</p>
<p>So far, its distributor Guidecraft has received two reports of tip-over incidents, including one report of a child who suffered minor contusions and abrasions.<span id="more-31230"></span></p>
<p>The 4-in-1 Dramatic Play Theater has a model number printed on a sticker affixed to the bottom of the centre crossbar and on the assembly instructions, it is: G51062. The toy is described by the CPSC as having: &#8220;&#8230; two interchangeable panels with different themes on each side: a puppet theater and diner, or a doctor&#8217;s office and post office. An assembled play theater measures 4-feet high by 3-feet wide and weighs about 46 pounds.&#8221;</p>

<p>These plush (soft toy) frog masks lack proper ventilation and when secured in place across a child&#8217;s face, it presents a suffocation hazard to the child.<span id="more-30475"></span></p>
<p>Around 3,400 units of this product are affected in this recall. So far, there have been no incidents or injuries reported.</p>
<p>The affected masks were made in China and predominately green in colour with yellow and red highlights. There are two eye cut-outs and a green elastic band with a fastener used to secure the mask at the back of the child&#8217;s head. The product&#8217;s UPC code 06626491474 is printed on a label attached to the mask.</p>
<p>Target exclusively sold this product in their stores from August 2011 through to September 2011 for about $1.</p>
<p>These masks should be immediately taken away from young children and returned to any Target store for a full refund.</p>

<p><strong></strong>The steel wire frame of the tent can break during use, this can cause sharp wire ends to protrude through the tent fabric, creating a risk of a laceration or puncture injury to a child playing with the tent.<span id="more-28902"></span><br />
There have been three reports worldwide of incidents where the wire frame has broken and protruded through the tent fabric. One such incident resulted in a minor injury.</p>

<p>Anyone who has an affected BUSA children’s folding tent should immediately take it away from children and return it to an IKEA store for a full refund.</p>
<p>The BUSA children’s tent has been sold in IKEA stores since 30th June 2011.</p>

<p>The hair on the Chloe and Sophie dolls may contain loops that are large enough to fit around a child’s head and neck, and the headband on the Audrey doll, if loosened, can form a loop that fits around a child’s head and neck. These loops can pose a strangulation hazard.<span id="more-26818"></span></p>
<p>The product was made in China, with around 81,000 dolls in the United States and 1,300 in Canada are affected by this recall. Pottery Barn Kids has received five reports of dolls with looped hair, including one report in which a loop of the Chloe doll’s hair was found around the neck of a 21-month old child. Thankfully, the child was not injured.</p>

<p>This recall involves soft dolls sold under the names Audrey, Chloe and Sophie. The dolls measure about 17 inches high and have hair made of yarn. Audrey’s hair is black, Chloe’s hair is dark brown and Sophie’s hair is blonde. The dolls are part of Pottery Barn Kids&#8217; Girl Doll Collection. The doll’s name can be found on a tag sewn onto her bottom.</p>
<p>The dolls were sold exclusively by Pottery Barn Kids stores nationwide, online at www.potterybarnkids.com and through Pottery Barn Kids catalogs from July 2006 to April 2011 for about $40.</p>
<p>Affected dolls should be taken away from children immediately, the looped hair of the Chloe and Sophie dolls should also be cut and the headband of the Audrey doll should also be removed to eliminate the hazard. Pottery Barn Kids may also be called for instructions on how to return the affected dolls for a merchandise credit.</p>

<p>The CPSC described the nature of the risk posed by the Metoo Clip-On Chair: &#8220;Missing or worn clamp pads allow the chairs to detach from a variety of different table surfaces, posing a fall hazard. In addition, when the chair detaches, children&#8217;s fingers can be caught between the bar and clamping mechanism, posing an amputation hazard. Also, user instructions for the chairs are inadequate, increasing the likelihood of consumer misuse.&#8221;</p>
<p>The recalled product is an infant/toddler chair with a nylon fabric seat and a metal frame that clamps onto tables using two metal vise clamps. The upper part of each clamp rests on the table top and has either a rubber pad on its underside or a rubber boot covering the clamp. The chair is sold in three fabric colors &#8211; red, black and navy. Chairs subject to this recall do not have black plastic spacers between the cross bar and the clamps.</p>
<p>Among the reports of injuries, two involved children&#8217;s fingers being severely pinched, lacerated, crushed or amputated. While the three other reports of injury involved bruising after a chair detached suddenly and a child struck the table or floor.</p>
<p>Around 54,000 of these high chairs are affected. In early May of this year the CPSC published advise about potential safety issues regarding this product.</p>
<p>The Metoo Clip-On Chair was sold via Buy Buy Baby, Target and Toys R Us, philandteds.com, Amazon.com, other online retailers and a variety of independent juvenile specialty stores from May 2006 to May 2011. The product was made in China and sold for between $40 and $50.</p>
<p>Phil &amp; Teds should be contacted in order to receive a free repair kit and revised user instructions. If you have previously received a repair kit with only rubber boots you should also stop using the chair and contact the company for the new repair kit.</p>
